• Your basic information, including your address and the last 4 digits of your social security number

• Basic information about your telephone and cell phone service, such as "Who is your service provider?" (who you pay your phone bill to)

• A Photo or a Scan of one of the following (to show proof of residency):

  • Driver's License​

  • State ID

  • Piece of mail (showing same name and address as you apply with)

• A Photo or a Scan of your recent landline phone bill, cell phone bill, or both (if you are applying for both landline and cell phone equipment through the program)
